If accepted as a student of the Bay View Conservatory, I agree to pay all fees, and abide by Conservatory and dormitory regulations. I agree not to bring any alcoholic beverages onto Association premises, not to smoke in any Bay View building or to possess or engage in unauthorized use of drugs while a student of the Conservatory. Conservatory authorities reserve the right to expel students for violations without returning any part of the fees paid. I hereby grant full clearance to the Conservatory to record and/or broadcast any performances by me, without compensation to me and to use photographs taken at Bay View in Festival publications. I understand a deposit of $350 is due upon receipt of notification of acceptance to the Festival to reserve my place and make me eligible for any scholarship consideration. I further agree to remain through the final Sunday evening performance of the Festival.

We accept credit card, check, and money order payments. No cash is accepted. If you are sending an application fee by mail, your application will not be processed until your application fee is received. For CC payments, you may call 231-225-8877 and leave info on the secure line or email cludwa@gmail.com separately with BV App Fee in the subject line. Either way, please give us your CC #, exp. date, mailing address, zip code, and 3 digit code on the back.
